Begin with the question and population
An evidence pack should explain what the institution examined, why that scope matters, and what the analysis can establish. Identify the product, application or account population, decision stage, time period, channels, and exclusions. Keep counts at each processing stage so a reviewer can reconcile the analytical population to the source systems. Document withdrawals, incomplete records, duplicate applications, and other exclusions without assuming they have the same meaning across products.
Preserve the analytical chain
Retain a controlled snapshot or authorized reproducible source reference, the data dictionary, transformation logic, code version, parameter settings, and quality checks. Record who ran the analysis and when. Store the executed outputs with the configuration that produced them rather than relying on a dashboard that changes each day. For each observation, state the comparison, the size and quality of the available population, and the assumptions used. Distinguish an observed difference from a conclusion about its cause. Document alternative explanations examined, unresolved questions, sensitivity checks, and the need for specialist review. A concise narrative should point to the supporting artifact instead of repeating a result without context.
Close the review loop
Assign an accountable owner for challenge, decisions, and any corrective action. Capture the reviewer’s questions, responses, approvals, and the rationale for further work or closure. If an input or method changes, preserve the previous version and explain the effect on comparability.
