Distinguish the types of discretion

Separate policy exceptions, model overrides, pricing adjustments, documentation waivers, and operational corrections. Each category can have a different purpose and approval authority. Record what the standard decision would have been and what changed. A single free-text field marked exception rarely provides enough information to evaluate patterns or determine whether the required approval process was followed.

Capture the decision context

Connect the adjustment to the application or account, product, channel, governing policy, original terms, revised terms, decision maker, approver, and timestamp. Use controlled reason categories with supporting notes where needed. Preserve the record of declined exception requests as well as approvals when relevant to the review. Reconcile the exception workflow to actual booked outcomes so the dataset reflects what happened, not only what was requested.

Review rates and outcomes together

Compare exception activity using meaningful denominators and documented segments. Review the direction and size of adjustments, repeated reasons, missing approvals, concentration by decision maker, and changes over time. Interpret differences in the context of product rules and available evidence. Do not assume that every exception is inappropriate, or that a low overall rate means individual decisions do not require attention.

Govern the response

Assign a review owner and a path for additional investigation, training, policy clarification, or control changes. Document the evidence supporting the selected response and the follow-up needed to assess it. If the institution changes its exception categories or thresholds, retain a mapping that explains the break in trend comparability.
